Cancellation Information:
A customer may terminate their account with MHO Networks by sending an email or fax or any written request using any of the following procedures:

* Via e-mail to support@mho.net

* Via facsimile to 303-226-8238

* Via U.S. Postal Service to:

MHO Networks
1190 South Colorado Blvd.
Denver Co 80246-3004

Written notification of termination must include login name, customer name, customer ID, address, and telephone number, and the effective date of termination of the account.

Accounts that are not terminated in accordance with the above procedures shall remain active and shall continue to incur monthly service charges until properly terminated. Term accounts may only be terminated in accordance with the terms of the applicable service agreement.
